Short-Term and Long-Term Rehabilitation

Easing your transition from hospital to home! We offer a wide spectrum of services that enhance recovery and focus on working to restore each client’s strength and abilities. The treatment we provide is as varied as the people we service. Some come to us after surgery and neurological traumas, while others are dealing with respiratory conditions or other ailments. Instead of only focusing on physical wellness, we believe that all dimensions of each person is important. Therefore we design care plans to include activities and programs that will enhance social, intellectual, spiritual, emotional, and physical aspects of each client. Symphony’s qualified case managers will work with you and your physician to create the best care options for your needs, lifestyle, and budget.

We provide medical interventions, including wound care and rehabilitation therapy, which are delivered in the comfort of your own residence. Experienced nurses, therapists and aides work with each client to maximize physical abilities, that improve their health and well-being. We assist with a variety of daily activities including bathing, and provide essential education and management of medications and medical conditions.

Palliative Care

A type of care focused on providing comfort and promoting quality of life through pain and symptom management, can be accessed while patients are still receiving aggressive, curative treatment. Our palliative care team, including nurse practitioners and other specialists working with each client’s physician to coordinate healthcare resources and provide assistance with decision making and advance directives.
